The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

Applewood Village Dentistry offers fee-for-service modern dentistry. We accept payments by Visa, MasterCard, American Express, Debit, cash or insurance checks, directly sent to us.

Dental Insurance

We accept most private as well as group insurance plans including Manulife, Sunlife, Great West Life (now called Canada Life), Empire Life, Blue Cross, Medavie Blue Cross, Manion & Wilkins, RBC Dental Insurance, Local 183 LIUNA, Local 506, Roofer's Union, City of Mississauga or Toronto or Trillium Hospital benefits, etc. 

Our Office no longer accepts Government Social Assistance insurance plans like Ontario Works, ODSP, CINOT and similar low reimbursement plans. With the extremely high costs of running a private practice post-COVID19, we can no longer work on fees set in 1996 and never revised after that! We apologize for this inconvenience.

For complex dental treatment plans that run in the many thousands of dollars, please consider using dentalcard.ca for financing or try using your line of credit.

You do not need dental insurance to get dental treatment. You can pay for it yourself, and you'd be surprised at how affordable modern dentistry actually is, considering the high value it provides!
